Please provide any information that you feel the instructor should be aware of prior to lessons. This may include, but is not limited to:

• Physical or medical conditions

• Learning differences or disabilities

• Behavioral considerations

• Other factors that may affect your child’s safety, comfort, or learning experience

This information will be kept confidential and is used solely to ensure a safe, supportive, and effective lesson environment.

Host Home / Host Neighborhood

If you are located within a 5–10 mile radius of Fuquay-Varina and are willing to host swim lessons at your home or within your neighborhood, please indicate your interest below.

Selecting this option means you are open to allowing lessons to take place at your property or nearby common area, helping to accommodate other students in your community. (minimum of 6 students)

A Registration Fee of $10 per child will be applied to all sign up's this year.

Payments

An invoice will be issued based on your lesson selections indicated above. All lessons must be paid in full prior to being scheduled. Please note that we do not offer prorated lessons or accept deposits.

We kindly ask for your patience during the scheduling process. Lessons are scheduled in the order that payment is received, and it may take up to 5–7 business days to process your payment and confirm your lesson times.

As a small business, we are currently unable to offer sibling or military discounts, though we hope to provide such options in the future.

By submitting payment, you acknowledge and agree to these terms.

Swim Lesson Conflicts and Scheduling Acknowledgment

I acknowledge that potential conflicts with scheduled swim lessons may include, but are not limited to:

• School schedules, including start/end times, early release, late starts, holidays, and special events

• Family vacations or travel plans

• Religious or cultural observances

• Extracurricular activities, private lessons, or other commitments

• Limited availability due to work, transportation, or caregiver schedules

Attendance, Lesson Consistency, and Cancellation Policy

Attendance and Consistency:Consistent attendance is essential for the effective learning, retention, and mastery of swimming skills. By enrolling, I acknowledge that adherence to my child’s assigned lesson dates and times is critical to their progress. I further acknowledge that I am paying for a reserved time slot, and that failure to attend may prevent other students from utilizing that time.

Lesson Changes:Requests to change lesson times must be submitted by the deadline provided at the time the enrollment spot is assigned. Any changes requested after this deadline cannot be accommodated. Only lessons canceled by the instructor in accordance with the policies below will be eligible for rescheduling. I understand that the registration form should be filled out accurately.

Instructor Communication: I may contact the instructor regarding lessons; however, a minimum of forty-eight (48) business hours is required for responses.

Refunds:No refunds will be issued for missed lessons, early departures, or tardiness.

Lesson Cancellation by Instructor:Lessons will only be canceled by the instructor in the event of pool closures due to unsafe or unforeseen circumstances, including but not limited to thunder, lightning, fecal contamination, unsafe chlorine levels, or other emergency pool shutdowns.

If a lesson must be stopped after ten (10) minutes due to unforeseen circumstances, the lesson will not be eligible for rescheduling or makeup, as the first ten minutes of instruction are considered the most critical portion of the lesson.

By enrolling, I acknowledge and agree to the terms of this attendance, cancellation, and refund policy and understand that adherence to scheduled lesson times and deadlines is essential to participation in the program.

Parent Observation and Drop-Off Policy

To maintain a safe, focused, and effective instructional environment, the following policy applies to all private pool lessons:

• Parents and guardians are not permitted to sit, remain on the pool deck, or stand near gates or fencing during lessons. All lessons operate as “drop-and-go” at the participant’s scheduled lesson time. Parents/guardians are required to remain off the pool deck and out of the child’s direct line of sight during the lesson and should wait in their vehicle.

• Parents/guardians may return to observe the final five (5) minutes of the scheduled lesson, provided they do so in a manner that does not disrupt instruction.

  • This policy is based on the instructor’s professional experience and is implemented to minimize distractions and promote student focus, safety, and learning efficiency. I acknowledge and understand that minimizing parental visibility during instruction supports improved student performance and reduces potential safety risks caused by distractions.

I further acknowledge that the parking area is visible to the pool area and that my child will not be outside of reasonable visual or auditory proximity.

• Early drop-offs are not permitted. Participants must arrive at their scheduled lesson time and not prior.

• Any participant arriving more than ten (10) minutes late will forfeit that day’s lesson, as late arrivals disrupt the instruction and safety of other students. No makeup lesson will be provided for arrivals exceeding ten (10) minutes late.

I acknowledge that failure to comply with this policy may result in cancellation of the lesson without refund.

I have read, understand, and agree to abide by the terms stated above.

Supervision of Non-Participating Children and Guests Policy

For the safety of all participants, siblings and other non-enrolled children are strictly prohibited from entering or being in or near the water at any time.

All non-participating children must remain under the direct supervision of their parent, guardian, or designated responsible adult and must remain within that individual’s immediate line of sight at all times. Unsupervised children may create distractions and unsafe conditions for instructors and students.

I acknowledge and agree that I am fully responsible for the conduct, supervision, and safety of any child or other individual I bring onto the property, including but not limited to siblings, relatives, guests, caregivers, or other invitees. If I permit any such person to be present on or about the premises, including engaging in any activity on the property, I assume full responsibility and liability for any and all injuries, damages, or losses that may arise from or relate to their presence or activities.

To the fullest extent permitted by law, I agree to release, indemnify, defend, and hold harmless the instructor(s), Swim With Jessie, LLC, and the property owner(s) from and against any and all claims, demands, causes of action, liabilities, damages, costs, or expenses arising out of or related to the presence, supervision, conduct, or activities of any person I bring onto the property.

I acknowledge that I have read, understand, and voluntarily agree to the terms above.

Photo, Video & Surveillance Acknowledgment and Consent

I hereby grant and authorize Swim With Jessie, LLC the irrevocable right and permission to photograph and/or record video or digital images of my child during lessons and related activities. I authorize Swim With Jessie, LLC to use such photographs, digital images, and/or video recordings for training, promotional, marketing, and/or educational purposes, including but not limited to printed materials, electronic publications, presentations, social media, and website publication.

I understand and acknowledge that the taking and distribution of photographs and/or videos is not guaranteed. I further understand that the primary responsibility of Swim With Jessie, LLC and its instructors is student safety and the quality of instruction, and that photographs and videos are provided as a courtesy and are not required as part of the lesson.

I understand that I may decline authorization by selecting “No” on the registration form. If I select “No,” no individual photos or videos of my child will be provided to me or used for promotional purposes. However, I acknowledge and agree that my child may still appear incidentally or in the background of photographs or video recordings taken during group lessons or general facility activities.

By selecting “No,” I understand that I will not receive any photos or videos of my child.

II acknowledge and understand that the taking of photographs and/or videos during lessons or related activities is a courtesy and is not a duty or obligation of the instructors or staff.

I understand that I may grant or deny permission for the use of photographs and/or videos of my child for promotional, marketing, or educational purposes. I further understand that even if I decline such permission, my child may appear incidentally or in the background of photographs or video recordings taken in group settings or common areas.

I acknowledge and understand that the premises are subject to continuous video surveillance for safety, security, and operational purposes. By enrolling, I consent to such surveillance.

Lesson Arrival & Departure Policy

To ensure our schedule runs smoothly and on time, the following policies are strictly enforced:

• Students must arrive at their scheduled lesson time.

• Do not enter the pool deck before your scheduled time.

• Our schedule is fully booked, and punctuality is required to keep all lessons running on time.

Late Arrival Policy:

• Lesson time will not be extended for late arrivals.

• If a student arrives 10 minutes or more after the scheduled start time, the lesson will be canceled and no makeup lesson will be provided.

Departure Policy:

• At the conclusion of each lesson, families must promptly exit the immediate pool deck area to allow space for incoming students.

Instructor Break Policy:

• Instructors are allotted a 5-minute break between lessons. This time is reserved for rest and preparation. It is not part of the lesson time and must be respected.

We appreciate your cooperation. Adherence to these policies allows us to provide the best experience for all students and families.

Failure to follow these policies will result in forfeiture of the lesson(s) without refund.

Illness Policy

To ensure the health and safety of all students and staff, the following guidelines apply:

1. Notification of Absence If your child must miss a lesson unexpectedly, please notify Swim With Jessie, LLC as soon as possible by:

   1. Messaging our Facebook page, or

   2. Sending an email to SwimWithJessie@gmail.com.

      This helps prevent concern regarding your child’s absence.

2. Health Guidelines for Attendance Do not bring your child to lessons if they:

   1. Have a fever or have had a fever within the past 24 hours

   2. Have vomited or experienced nausea within the past 24 hours

   3. Have missed school due to illness

   4. Have any viral skin infections or contagious conditions, including but not limited to hand, foot, and mouth disease

   5. Serious Wounds or bleeding

   6. Are exhibiting symptoms of viral illness, including a persistent or strong cough

   7. Tested positive for Covid, Flu A, Flu B, Strep, etc.

3. Lesson Cancellation for Health ReasonsThe instructor reserves the right to pause or cancel a lesson at any time if a child is deemed too unwell to participate safely. Lessons canceled for health reasons by the instructor will not be eligible for a refund or makeup.

Important: We have an instructor with an autoimmune condition. To protect their health and the health of all students, it is essential that these illness policies be strictly followed.

By enrolling, you acknowledge and agree to adhere to this policy to ensure a safe and healthy environment for all participants.

Diet Policy

For the safety and comfort of all students, please adhere to the following guidelines regarding food and drink prior to swim lessons:

• Do not provide your child with any food or drink for at least two (2) hours before their lesson.

• Avoid giving dairy products or foods high in fat within the two (2) hours prior to the lesson.

These guidelines help prevent discomfort, nausea, and potential safety risks during instruction.

Swim Attire Policy

To ensure the health, safety, and comfort of all participants, the following swim attire requirements apply:

1. Swim Diaper Requirements

   1. Any child who is not fully toilet trained for at least six (6) months must wear two (2) layers of washable swim diapers.

   2. Disposable swim diapers are not acceptable, as they do not contain fecal matter reliably.

   3. A swimsuit may be worn over the two layers but does not count as a layer unless it contains a built-in swim diaper, which may serve as one of the required layers.

   4. A spare washable swim diaper must be available at all times in case of an accident.

2. Compliance and Safety

   Children not in proper swim attire will not be allowed in the water.

   If a child fails to comply and fecal matter enters the pool, a $100 fee will be charged to cover pool closure, additional chemical treatment, and lost instructional time.

3. Health and Safety Priority

   The health and safety of all Swim With Jessie, LLC students is of the highest priority. Adherence to these attire guidelines is mandatory, and we will not compromise safety.

Agreement and Compliance

By enrolling my child in lessons with Swim With Jessie, LLC, I acknowledge and agree to the following:

1. Binding AgreementI understand that this is a binding contract. By providing an electronic or handwritten signature, I agree to comply with all policies, rules, and terms outlined in this registration, including, but not limited to, attendance, illness, supervision, attire, diet, and safety policies.

2. Responsibility for Children and GuestsI acknowledge that I, or my designated agent, am fully responsible for the safety and well-being of my child and any other children or guests that I bring to the premises.

3. Release of LiabilityI, on behalf of myself, my child, and any guests I bring to the premises, hereby fully and forever release, discharge, and hold harmless Swim With Jessie, LLC, its representatives, the property owner, and the business owner, Jessica Cookson, from any and all liability, claims, demands, or causes of action, including but not limited to injuries, illness, or property damage, arising from participation in swim lessons or activities on the premises, whether caused by the negligence of the released parties or otherwise.

4. Financial ResponsibilityI agree to be financially responsible for any damage to property caused by my child, myself, or any other children or guests that I or my designated agent bring to the premises.

5. Video SurveillanceI acknowledge that the property is monitored by video surveillance for safety and security purposes.

6. Acknowledgment of Rules and DurationI acknowledge that I have read and understand all rules, policies, and procedures outlined in this registration. I agree to follow them and understand that this agreement remains in effect for the entire duration of my child’s participation in lessons with Swim With Jessie, LLC, including all future lessons for as long as my child is enrolled.

Agreement and Compliance

I understand that this is a binding contact. I understand that by clicking the box and providing an accurate signature, I agree to the following terms. I understand that I, myself or my designated agent shall be totally responsible for their safety and well-being of my child or any other children that are with me. I completely and totally release SWIM WITH JESSIE, LLC and their representatives, the property owner and as well as, the business owner, Jessica Cookson, from any potential liability for any possible personal injuries and/or personal property damage suffered by any child or adult that I or my designated agent brings to the residence while on premises. I agree to be financially responsible for any damages to property that my child and or any other children that are with me, I, myself or my designated agent, as well as myself and or designated agent cause. I understand that the property is video and surveillence monitored. I completely and totally release Jessica Cookson, property owner and Swim With Jessie, llc representatives from any potential liability for any possible personal injuries and/or personal property damage suffered by any child that I or my designated agent brings to the residence while on premises. By clicking below, I hereby release and discharge Jessica Cookson, Swim With Jessie, llc, from any and all liability, claims, demands, or causes of action that may hereafter have for injuries or damages arising out of my participation in the Swim, including but not limited to losses caused by the negligence of the released parties. I ACKNOWLEDGE I HAVE READ THE ABOVE RULES AND I ACCEPT AND AGREE TO FOLLOW THEM. I ALSO ACKNOWLEDGE THAT THIS IS A BINDING CONTRACT THAT WILL BE VALID FOR THE ENTIRE DURATION OF LESSONS FOR AS MANY YEARS AS MY CHILD IS IN LESSONS.
